Send an e-mail to  yourstory@flukenetworks.com no later than, 11:59 PM US Pacific Time August 31, 2008 with “OptiFiber Your Story Contest Entry” in the Subject Line.  In the body of the e-mail provide the following points of information: 

1. Your complete name, street address, city, state/province, zip/postal code, country, phone number and e-mail address.
2. The name of your employer and your job function/title.
3. The number of nodes in your work environment.
4. The model number of your Fluke Networks OptiFiber OTDR and approximately when you purchased it.  (Applicable models for the purposes of Contest Submission include: OF-500-35/50M, OF-500-02, OF-500-02/50M, OF-500-10, OF-500-10/50M, OF-500, OF-500-45, OF-500-01, OF-500-01/50M, OF-500-35, OF-400-03, OF-500-13, OF-500-03, OF-500-15/50M or OF-500-15.
5. How frequently you use your Fluke Networks OptiFiber OTDR and the typical use.
6. A detailed description of the most critical or significant problem you have solved using your Fluke Networks OptiFiber OTDR. (There is no limit to the length of your description and in-depth detail is encouraged.)
7. Attach a color photograph to your e-mail, which shows you and your OptiFiber OTDR.  The photograph should be a .jpg format and should not be larger than 5 MB.
